Warnings and Precautions :
Extended usage of corticosteroids may raise intraocular pressure in vulnerable people, causing glaucoma, which damages the optic nerve and impairs vision fields and visual acuity. When glaucoma is present, the use of steroids should be cautious, and regular checks of intraocular pressure should be made. Reports of extensive or prolonged topical ophthalmic corticosteroid use have indicated the development of posterior subcapsular cataracts. When long-term, frequent application of high-dose topical steroids is undertaken, especially in infants and children, one should take the potential for adrenal suppression into consideration. Patients may have temporary blurred vision after installation, making it difficult for them to use machines or drive.Until their vision clears, people who are impacted should stay away from operating machinery or driving.

Mechanism Of Action :

Prednisolone acetate glucocorticoid with three to five times the anti-inflammatory potency of hydrocortisone based on weight is  Glucocorticoids block the acute inflammatory response’s edema, fibrin deposition, capillary dilatation, and phagocytic migration in addition to capillary proliferation, collagen deposition, and scarring.
Safety Advice Pred Forte ophthalmic suspension :

  • Wash your hands before using eye medication.
  • Shake the eye drops well just before each use
  • Follow all the instructions as directed by doctor or physician
  • Keep bottle or tubule tightly closed when not in use.

Drug Interactions :
Co-treatment with CYP3A inhibitors may raise the risk of systemic corticosteroid-related side effects, even if the systemic exposure from topical ophthalmic corticosteroid therapy is anticipated to be minimal. Pred forte ophthalmic solution may interact with acarbose 25 mg or 50 mg. So it is recommended to monitor the blood glucose level regularly as cases of high blood sugar levels have been seen in some patients. Special Precautions: Topical steroids have been observed to cause perforation in diseases that cause the cornea to thin.. Topical steroids can either suppress or increase acute purulent or swollen eye infections. Extended use raises the risk of subsequent eye infections by suppressing the human immune response. Fungal invasion should be suspected in any prolonged corneal ulceration where a steroid has been or is being administered, since fungal infections of the cornea are especially likely to occur coincidentally with long-term local steroid treatments. Fungal cultures need to be handled carefully. Steroid use following cataract surgery has been shown to promote bleb development and delay healing. Soft contact lenses may absorb the preservative in pred forte benzalkonium chloride and become discolored as a result. It is recommended that patients who wear soft contact lenses take out their lenses before administering the solution and wait for at least fifteen minutes before putting them back in. It is important to take precautions to keep the bottle or tube tip away from the eyes and other surfaces in order to prevent contamination or eye injuries. If other people use the bottle or tube, an infection could spread. When not in use, keep the bottle or tube tightly closed. Systemic and topical corticosteroid use has been linked to visual disturbances. If a patient exhibits symptoms like blurred vision or other visual disturbances, consider evaluating for potential causes, such as gluacoma, cataracts, or rare diseases like central serious chorioetinopathy (CSCR), which has been linked to systemic and topical corticosteroid use.
